University Hospitals Expands Personalized Primary Care Offerings with New UH Select Westlake Practice

Membership-based concierge care provides enhanced access, extended appointments and a stronger physician-patient relationship

University Hospitals has opened a new UH Select practice in Westlake, bringing personalized, concierge care to patients on Cleveland’s West Side. Located on the UH St. John Medical Center campus at 29099 Health Campus Drive, Suite 170, the practice is welcoming new patients and is led by Cynthia Caja, DO, and Jason Frazier, DO.

Through its membership-based model, UH Select offers enhanced access to care, including same-day or next-day appointments, extended office visits, 24/7 access to patient navigation services and exclusive membership perks. Some key benefits include acupuncture and massage therapy by licensed professionals at UH Connor Whole Health, an on-site clinical pharmacist, access to a registered dietitian, fitness evaluations, behavioral health support and health coaching.

“UH Select allows us to spend more time getting to know each patient and developing care plans tailored to their unique health goals,” said Dr. Frazier. “We are building stronger relationships and providing enhanced access so we can deliver a more personalized experience that helps patients stay healthier.”

The concierge program emphasizes preventive care, wellness and convenience while offering patients direct access to a dedicated care team. UH Select is open to individuals 3 years of age and older. Annual membership fees allow UH Select doctors to limit the size of their practice and provide each patient with dedicated attention and enhanced health services in a high-touch practice environment.
