Foundations Health Solutions project will expand access to skilled nursing, rehabilitation and long-term care services in Lorain County
Construction is officially underway on Sandstone Crossing, a new skilled nursing and rehabilitation center in Amherst Township.

The new center, supported by Foundations Health Solutions, will bring a modern, resident-centered environment for individuals needing short-term rehabilitation, skilled nursing services and long-term care to the Amherst Township and greater Lorain County community.
“We’re excited to see Sandstone Crossing taking shape and to make this investment in Amherst Township,” said Rob Peterson, Administrator of Foundations Health Solutions. “This center is being built around a simple goal: creating an exceptional place for residents to receive care and an exceptional place for caregivers to work.”
With construction now underway, Sandstone Crossing is being designed to meet the needs of today’s residents, families and healthcare professionals. The center will feature 98 private resident rooms, modern rehabilitation and therapy spaces, welcoming common areas and amenities designed to promote recovery, independence and quality of life.
Sandstone Crossing is expected to provide a full range of post-acute and long-term care services, including:
- Skilled nursing care
- Short-term rehabilitation
- Physical, occupational and speech therapy
- Long-term care
- Wound care and specialized clinical services
- Restorative nursing
- Social services and care coordination
- Activities and life-enrichment programming
Beyond expanding local access to care, Sandstone Crossing will bring new employment opportunities to Amherst Township and the surrounding area. The center will employ nurses, nursing assistants, therapists, dietary and environmental services professionals, administrators and other members of the caregiving team.
“Buildings are important, but ultimately it’s the people inside them who make the difference,” said Peterson. “As construction progresses, one of our biggest priorities will be building a team of caregivers who share our commitment to taking care of residents, families and one another.”
The project represents a continued investment by Foundations Health Solutions in Ohio’s post-acute and long-term care infrastructure and reflects the organization’s focus on strengthening local communities through quality care, strong leadership and a caregiver-centered culture.
