Avon Place, a proud member of the Foundations Health Solutions Family (one of the largest skilled nursing providers in the country) is now serving the community with a new Dialysis Center.
The Dialysis Den, located within Avon Place at 3290 Detroit Road in Avon, is the only Skilled Nursing Center in Avon offering dialysis.
Partnering with Renew Dialysis, the Avon Place Dialysis Den is dedicated to the highest levels of patient care. Renew Dialysis was founded by a group of skilled nursing, long-term care and renal health professionals who were determined to provide a better way administering dialysis treatment to residents of nursing facilities.
The nine-chair unit includes bariatric chairs to dialyze patients up to 500 pounds. Each patient’s nephrologist may follow at Avon Place, and two credentialed nephrology physicians serve the unit.
Respiratory Care is one of the many services now offered at Avon Place.
Renew provides case management, on-site RN, Dialysis Technicians and Lab Services. All Avon Place staff are trained to care for dialysis patients and their specific medical needs.
Benefits of five weekly treatments include in-house care (avoiding transportation), shorter run times, fewer missed appointments, increased life expectancy, flexibility in diet and reduced re-hospitalizations.
The Dialysis Den represents an addition to the many existing services now serving the community at Avon Place. Existing offerings include Ventilator Dialysis, In-House Dialysis, Ventilator and respiratory care, 24-hour skilled nursing, Physical, Occupational and Speech Therapies, and Pathways Secure Memory Unit. Pathways offers personalized programming with the goal of enabling residents to function at their maximum ability and enjoy a meaningful relationship with peers, friends and family.
